The City of Memphis continues to respond to the coronavirus (COVID-19)
pandemic. Thank you for everything you do to keep yourself and everyone around you safe and healthy. Getting a COVID-19 vaccine is an important step needed to prevent getting sick with the COVID-19 disease. This is also an opportunity for you to serve as a role model in your community. By getting vaccinated, you can positively influence the decisions of coworkers, friends, and family.

VACCINATION INCENTIVE FORM

The City of Memphis continues to respond to the COVID-19 pandemic. Employees are encouraged to receive the vaccine now that it is available to all groups. As an incentive, employees who receive the full manufacturer-recommended dose(s) of the COVID-19 vaccine can earn a $200 VISA gift card. Please click below to complete the form.
